Form5 TIMELINE
FEBRUARY 2015
At age 15, Founder Aaron Westbrook 3-D prints his first device at New Albany High School, laying the foundation for a path to empowering others.
Aaron documents this journey on his blog ‘Alive With Five’.


SEPTEMBER 2016
Aaron initiated ‘Forming 5’ – his high school senior seminar project.
AUGUST 2017
Established Form5 Prosthetics as a 501(c)3 non-profit organization.


NOVEMBER 2019
Hosted the inaugural CO-FAB Workshop. A workshop that allows those with limb differences to work directly with designers, engineers, and medical professionals to turn their imagination into innovation.
March 2020
Adopted 2020-2023 Strategic Plan (thanks to Inogize) and expanded Form5’s Board of Directors from 4 to 8 members.


October 2020
Hosted the second CO-FAB Workshop. A workshop that allowed three individuals with limb differences to work directly with designers, engineers, and medical professionals to innovate devices for rock climbing and riding a bike. Along with, developing custom shoes.
August 2021
Form5 recipient, Jodie Pierpoint and Form5 CEO, Aaron Westbrook rode in Pelotonia. Riding 20+ miles, further than they ever have in their lives.
